HACC (Home and Community Care)

Harold Hawthorne Community Care & Day Centre is incorporated within Harold Hawthorne Senior Citizens and Homes Incorporated. It is a non-government community based Organisation and is managed by an annually elected Board of Management Sub-Committee.

Harold Hawthorne Community Care and Day Centre provide services to frail, elderly people and adults with disabilities living in Victoria Park, East Victoria Park, Carlisle, Lathlain, St. James and parts of Bentley including Rowethorpe Village and Bentley Park (formerly Swan Village of Care).

We are funded under the Home and Community Care (HACC) Program, a joint project of both State and Commonwealth Governments. Eligible clients pay a small fee for each service they receive. A fee policy schedule is given to all consumers detailing the cost of each service. The cost of each client's services is dependent on their income level.

Our mission is to maintain and improve the quality of life of frail aged and younger people with disabilities, in their own homes, and prevent premature or inappropriate institutionalisation.

Harold Hawthorne Community Care Services encourages clients and their carers to make choices in their everyday living.

Service provision is given with the aim of maintaining the client's rights to self determination, dignity, respect, privacy and confidentiality.

 

How does the service operate?

The annually elected committee provides management services. Paid HACC staff carry out the day to day operations and Volunteers provide other services such as Transport and assistance in the Day Centre. HACC staff are trained and, where necessary, certified to carry out services such as Personal Care.

•  Counselling, Information and Advocacy

 

•  PERSONAL CARE : Experienced Carers provide a range of services including assistance with:

*   personal care tasks such as showering, dressing and grooming.

*   shopping, banking and bill paying.

*   Doctor and Hospital appointments.

•  DOMESTIC ASSISTANCE: Home Help Carers provide services which include assistance with:

*   basic house work such as vacuuming, mopping, sweeping, cleaning bathrooms and toilets.

*   changing bed linen.

*   washing and ironing.

*   cleaning out fridge and cupboards.

*   other tasks which fall within the HACC funding guidelines can be arranged through Client Services

 

•  TRANSPORT: Volunteer Drivers provide local transport.

Taxi Vouchers are available to eligible clients for medical appointments.

We refer clients to HACC Brokerage Transport for more extensive transport assistance.

Please Note : At least 2 days notice is preferred for transport bookings.

 

•  HOME MAINTENANCE/GARDENING: Our Gardeners provide a basic maintenance gardening service which includes tasks such as lawn mowing, pruning, weeding, digging over gardens and applying fertilisers. We can also change light globes and replace tap washers.

Please Note : Jobs such as tree lopping, plumbing or electrical work CANNOT be done.

ADULT DAY CENTRE (Centre Based Day Care)

The Adult Day Centre is open 6 days per week, offering a safe and secure environment where frail aged & younger people with disabilities can enjoy the company of others while engaging in activities and outings.

Transport is provided to and from home by car or the fully equipped Day Centre bus.

Trained, caring staff provide programs to meet the needs of a wide range of clients including one day exclusively for younger people with disabilities.

Specific day programs are offered for clients with dementia.

How to obtain assistance?

If you would like assistance or further information please contact the office on 9361 3891 or 9362 3891 where the Program co-ordinator or one of the staff can answer any questions you may have. If you wish to proceed and are eligible to receive services, the Clients Services Co-ordinator will visit you in your own home. Your eligibility and service needs will be assessed and you will be advised of the full range of services we offer and the cost of each service.
